This is a hybrid position requiring both on-site and remote work. You will be working with a team to assist in connecting patients with follow-up care, complete chart reviews and follow-up based on a well-established protocol system. The team works daytime hours, typically 12-hours with occasional weekends (approx once every 8-10 weeks). This position is a collaborative role working closely with ED providers and staff. Must have good communication skills and highly motivated as this is an autonomous position.

What We Require:

Education: Graduate of an accredited School of Nursing required. Bachelor's degree Preferred.

Experience: Minimum of 3 years RN experience required. At least 2 years experience in relevant specialty or department, as defined by the department, desired (ex: hospital setting, vascular, rehab, long term care, home health, cardiology, ambulatory, surgery).

Licensure, certification, and/or registration: Current VA RN license required. Must successfully complete orientation and competency requirement.

Must obtain and maintain appropriate certifications as defined by the department.

  • BLS-HCP required within 1 year of hire or at time of hire
  • ACLS, Conscious Sedation, CCRN, ONC, Pain Management and other modality specific certifications (ex: Cardiology, Surgery, Orthopedics, Vascular) certifications desired.

Valid Virginia, or state specific, drivers license required for employees working in Home Health or departments where the work requires traveling.

Other Minimum Qualifications: Effective interpersonal, communication, and team-working skills required. Strong critical thinking, conflict resolution, organization and effective judgement abilities required. Must successfully complete orientation and competency validation requirements. Must be able to meet essential job functions. Designated areas may require Knowledge of Medicare/Medicaid and other third party payor regulations

* Position is deemed emergency personnel during emergency weather conditions.
