Bone Marrow Transplant Clinical Pharmacy Specialist Job Responsibilities:
- Facilitates the development and implementation of treatment guidelines, protocols, formulary changes and critical pathways.
- Facilitates the identification, resolution, implementation, and communication of cost-effective medication use issues in collaboration with the medical staff.
- Provides clinical consultation and clarification to practitioners and patients and suggests appropriate and cost-effective therapeutic alternatives to medical staff as needed.
- Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, adverse drug reactions and appropriateness of drug/dose.
- Prepares and presents drug reviews, MUE criteria, antibiotic sensitivity reports, adverse drug reaction reports and pharmacy intervention summaries to the Pharmacy and Therapeutics Committee.
- Works under a collaborative practice agreement with provider team.
Bone Marrow Transplant Clinical Pharmacy Specialist Additional Requirements:
- Work schedule: 8-hour shift (Monday - Friday days, various shift.)
- Education: Doctorate of Pharmacy (PharmD).
- Experience: Two (2) years of clinical oncology experience. PGY1/PGY2 Oncology residency required.
- Certification: Licensed Pharmacist in the state of VA. Board certified Oncology Pharmacist or certification obtained within two (2) years of employment.
