Social Worker 1 Psychiatry (LSW required) - 10524 Euclid Ave.

Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans.

What You Will Do

• Provides psychosocial assessment / intervention to assist patients/families. 40%
  

  * Provides age specific care to patients/families in all age groups.

  
  
• Develops and implements a comprehensive plan based on identified psychosocial needs and/ or discharge needs. 45%
  

  * Develops plans with attention to individual patient and family goals

  Provides linkage to community resources to facilitate optimal care

  
  
• Communicates/collaborates with the work team using a multidisciplinary approach 15%
