About the Department

The Care Management Team at Lakewood Health System plays a vital role in improving patient outcomes through chronic condition management and patient care coordination. The department works closely with patients, providers, and multidisciplinary teams to deliver compassionate, high-quality care that supports patients' long-term health and well-being.

Mission for Position

The LPN/CMA Clinical Care Coordinator will support the Care Management Team, including our Advanced Practice Provider (APP), to manage patients with chronic conditions and ensure a patient-centered approach to care coordination.

Duties & Responsibilities



  • Foster strong, trusting relationships with patients and families to enhance care delivery and engagement.
  • Educate patients and families on chronic condition management, self-care techniques, and treatment adherence.
  • Collaborate with healthcare providers and multidisciplinary team members to deliver coordinated, patient-focused care.
  • Act as a patient advocate by addressing concerns and facilitating access to appropriate support resources.
  • Support rooming responsibilities for Care Management APP, including taking vital signs, updating patient histories, and conducting preliminary assessments.
  • Administer medications and perform clinical tasks under the direction of the APP or supervising provider.
  • Maintain accurate documentation of patient interactions, assessments, and care plans in the electronic health record (EHR) system.
  • Coordinate referrals, follow-up appointments, and external services to ensure comprehensive care.
  • Support patient panel management to meet value-based contract requirements by coordinating care, closing quality gaps, and ensuring compliance with contract benchmarks.
  • Contribute to the continuous improvement of care management processes, participating in quality initiatives and multidisciplinary meetings.



Position Type



  • This is a full-time, non-exempt position.


Qualifications

Qualifications



  • Current LPN or CMA certification and licensure in good standing within the state of Minnesota.
  • Minimum of 2 years of clinical experience in a healthcare setting, preferably in a care coordination or chronic condition management role.
  • Strong clinical skills with the ability to perform patient assessments, administer medications, and support provider care delivery.
  • Knowledge of chronic condition management and evidence-based care practices.
  • Excellent communication and interpersonal skills to effectively collaborate with patients, providers, and team members.
  • Proficiency with EHR systems and ability to document patient interactions accurately.
  • Strong organizational skills and the ability to manage multiple priorities in a fast-paced environment.
  • Commitment to patient-centered care and a compassionate approach to patient advocacy.



Ideal Candidate



  • Previous experience in care management or clinical coordination preferred.
  • Certification in chronic care management or care coordination training preferred.
  • Excellent teamwork and collaboration skills.
  • Strong problem-solving abilities and attention to detail.
  • Ability to work independently and manage a diverse workload.
