Nurse Practitioner Physician Assistant Neurology Sterling Heights and Detroit Full time Days

APP will work at two locations. 75% of time will be at our Lakeside facility on Hall Rd in Sterling Heights. 25% of the schedule will be at Henry Ford Hospital in Detroit.

• APP will be in the Vascular/Stroke Division of Neurology and will be based in the clinic. Primarily, APPs will see ischemic stroke and TIA patients after a hospitalization (i.e. stroke hospital follow-up visits); possibly some neuro endovascular follow-up.
• APP will work at Lakeside and Detroit where we have TJC-certified Thrombectomy- Capable Comprehensive Stroke Centers, demonstrating clinically excellent stroke care based on the most current national clinical practice guidelines.
• APP will become a part of a team caring for complex patients with stroke, contributing to excellent outcomes across the care continuum; providing excellent outpatient care that complements the inpatient care for effective secondary stroke prevention and recovery support.
• The Specialty/Invasive Physician Assistant (PA) or Nurse Practitioner (NP) will work in a collaborative, multidisciplinary environment
• APP will be responsible for the assessment, diagnosis, treatment, education, consultation, and care coordination as a first line provider to patients and their families.
• Provides outpatient medical care. Works in collaboration with attending physicians, advanced practice providers, consultants, house officers, nurses, and other members of the medical care team to ensure that patient outcomes are achieved safely and efficiently.
• Specific procedures as per credentialing as indicated by training and experience.
