Position Summary The Salvation Army Northern Division Anoka 360 Life Center is doing the most good by providing holistic, person-centered services for individuals and families in need. These services include food, clothing, utility, rent, and housing support, short- and long-term case management, spiritual support, and connections to community partners. The Case Manager for the Family Homeless Prevention and Assistance Program (FHPAP) guides participants in navigating housing and community resources through targeted case management services. The Case Manager empowers participants experiencing homelessness and housing instability to secure and maintain housing, increase their self-sufficiency, and connect to their communities. Additionally, the Case Manager provides direct assistance to participants and supports Coordinated Entry efforts within the county by completing participant assessments and engaging in community work. $23.00/hour, 40 hours/week This is a time-limited position that will end on 9/30/2025. Supervisory Responsibilities None. Travel Requirements May occasionally drive agency vehicle locally to support the mission if MVR check cleared. May be required to use public transportation to support the mission if unable to drive on behalf of The Salvation Army.
