Position Summary

Reporting directly to the Medical Director of the NCCP, the Clinical Coordinator III (RN III), assists the clinical team with patient care coordination, electronic medical record (EMR) chart review, and the administrative management of the Neonatal Comfort Care Program and its projects. These duties include assisting the medical team with the planning and implementation of palliative/comfort care, facilitating communication between patients and clinical team, leading interdisciplinary rounds and supporting the education of the nursing staff. The Clinical Coordinator III (Registered Nurse III) will work closely with multiple providers as well as other clinical and administrative staff to deliver an excellent patient experience that contributes to positive health outcomes for patients and a work culture that represents our values.

Responsibilities

• Works with the care team to coordinate patient encounters in the outpatient setting.
• Provide interdisciplinary care coordination.
• Function as first responder to referrals, community queries and patient questions.
• Check in with Comfort Care patients in the postpartum period to evaluate need for additional support.
• Review data related to patient health status and update medical team regularly, including weekly rounds.
• Review and manage communications from internal and external "customers."
• Participate in interdisciplinary consultations.
• Assist in planning the annual international training and additional national and international educational workshops.
• Support the Medical Director in administrative coordination of the program.
• Other duties as assigned and appropriate to scope, education, and/or training.

Minimum Qualifications

• Requires a Bachelor's degree in Nursing or equivalent in education and experience.
• 4 years related experience. Clinical rotation experience associated with BSN degree programs will be applied toward related experience requirements.
• BLS certified.
• Current New York State RN License.
• Ability to work indoors in an environmentally-controlled environment with a degree of dust, noise, odors, etc., without undue discomfort.
• Ability to perform sedentary work that primarily involves sitting or standing for long periods of time.
• Travel between clinical sites may be required depending on need.
• Duties performed routinely require exposure to blood, bodily fluid, and tissue. Incumbent in expected to perform and/or/assist in emergency care and/or first aid as needed.

Preferred Qualifications

• 4 years of work experience in the Neonatal ICU in a Level IV NICU.
• Proficient with EPIC.
• Strong organizational and time management skills.
• Shared mission with the NCCP.
• Strong interpersonal skills; Proactive.
