A Brief Overview Assist CHOP Career Path program participants (with special needs) to determine appropriate employment possibilities according to strengths and interests within the CHOP system, or in the community.
After job placement, the job coach will be responsible for maintaining an on-going relationship with the worker (program graduate) and supervisor to help facilitate successful employment.
What you will do
- Demonstrate knowledge and abilities in the areas concerning job coaching, employment and special accommodations for successful employment in order to facilitate the training and integration of a program participant with disabilities in the worksite trial rotation and/or competitive work environment.
- Provides individual support for participant at the worksite rotation or competitive job which includes: steps of the job coaching plan, building natural supports, etc.
- Trains participants in the areas of grooming, hygiene, communication, interviewing, and behavior as they relate to successful employment.
- Provides travel training to job site if necessary. Ability to work off-site, away from CHOP Main, in other CHOP and community locations, with minimal supervision.
- Willingness to provide job coaching as needed for shifts other than day shifts, including evening, overnights and weekends.
- Responsible for maintaining appropriate supervision of the participant with special needs, consultation with staff, accurate documentation for clinical purposes, and advocacy for people with special needs.
- Maintain appropriate progress and hospital job records.
- Evaluation of job performance of worker.
- Attend trainings and workshops.
- Meet regularly with the participant and supervisor to go over issues and successes; develop strategies with student/worker and supervisor to lessen, eliminate problem areas.
- Identify potential employment and possibilities for advancement
- Assist in removal of barriers and implementation of a more accessible environment.
- Co-Lead Group sessions in the areas of grooming, hygiene, communication, interviewing, and behavior as they relate to successful employment.
- Promote a positive, professional work environment with students/workers.
- Actively supports the mission, goals and priorities of The Children's Hospital of Philadelphia.
- Serves as a role model to the program participants to demonstrate positive work ethic.
- Demonstrates cultural sensitivity in all work performed.
- Appropriately exchanges constructive feedback with peers, colleagues and supervisors.
- Collaborates with others in the promotion of health through education and wellness initiatives.
- Acts in advocacy role to develop and improve opportunities for participants and workers with special needs
- Plans, develops and implements interventions and systems that improve success of the participants and workers
- Oversees and interacts with participants/workers with special needs in the areas of job shadowing and job trials.
- Develop and enhance professional practice skills.
- Recognize areas of strength and areas for development. Assists in plan creation for supervision, consultation and/or learning.
- Utilize team members, peers and supervisory staff in consultation for professional development.
- Incorporate new knowledge and feedback into practice.
- Practice professional writing skills for progress notes and report writing.
Education Qualifications
- High School Diploma / GED Required
Experience Qualifications
- At least one (1) year working people with cognitive and/or physical disabilities Preferred
- At least one (1) year job coach experience Preferred
Skills and Abilities
- Basic proficiency with office software (Microsoft Office).
- Basic knowledge of applicable legislation, federal and state, and hospital policies.
- Basic knowledge normal developmental behavior of adolescents and all areas of development of people with special needs.
- Basic knowledge of data management.
- Ability to develop trusting relationships with participants, workers, patients, and families.
- Ability to effectively use verbal and written communication to share performance and psychosocial information.
