The Coordinator for the Henry Ford at Home Outreach Program is responsible for driving referral growth from skilled nursing facilities, physician offices, senior communities, and more. This role involves 90% travel within Wayne and Northern Monroe Counties, building relationships, and developing strategies to increase growth, admissions, referrals, and awareness of our services through cold calls and networking.

The ideal candidate is a dynamic, high-energy individual with excellent communication skills and experience working with referral sources.

GENERAL SUMMARY

The Coordinator - Henry Ford at Home Outreach Program is responsible for:



  • Generating business and meeting targets from assigned territory.
  • Develop and implement business building and relationship building strategies to increase admissions.
  • Develop and maintain community relationships to promote Henry Ford at Home services.
  • Maintain referral sources, including physicians, hospitals, senior retirement communities, skilled nursing facilities and the community.



**Primary responsibilities will be in the Community (Wayne and Northern Monroe counties) primary focus being SNF's and physician offices. **

EDUCATION REQUIRED:



  • Minimum of an Associates degree in business, communications, marketing or related health care field from an accredited institution required.


LICENSE/CERTIFICATIONS:



  • Registered Nurse or Social Worker preferred.


EXPERIENCE REQUIRED:



  • Requires a minimum of three years of experience in health care service.
  • A minimum of one-year experience in business development preferred.


SKILLS REQUIRED:



  • Requires a competitive mindset to meet and exceed business objectives.
  • Requires commitment to Henry Ford at Home philosophy of care.
  • Requires strong interpersonal communication, and verbal and written communication skills.
  • Requires the ability to work independently as well as with teams.
  • Demonstrates a high confidence level to interact with health care professionals at all levels.
