Job Title: Licensed Practical Nurse I

Duration: 4+ Months to Long term


Location: Hammonton, NJ 08037

Pay Rate: $42.50/hr.

Shift Timing: 8:00Am- 4:30 Pm

Job Description:

  • When an admission arrives and the primary staff member for COVID testing is not in the facility, the nurse will be required to conduct COVID testing. (utilize shared calendar for reference).
  • In instance where a staff is COVID positive, the Nurse will report accordingly, conduct well checks and issue appropriate emails
  • Perform any medical testing deemed necessary by the Superintendent to enhance programs.
  • Ensure Resident medical certification is complete and updated annually as required, monitor files for expired medical release forms, log updated immunizations, medications, medical diagnosis
  • Collaborate w/ the Programs Staff along w/ a variety of community resources to enhance healthcare choices for Facility.
  • Perform the role of a veteran's advocate to promote wellbeing of the veterans by assessing treatment needs and challenges.
  • Accompany veterans to appointments when necessary.
  • Utilize shared calendar daily, track accepted/denied transportation requests, to ensure medical transportation coverage, provide transportation for medical appointments and communicate with Programs Staff.
  • Assess and evaluate veterans' responses to services, treatment and medications provided.
  • Provide medical case management prior to discharge
  • Collaborate w/ other Program Staff members in problem solving regarding veteran's care and links veterans to ongoing medical, behavioral health and supportive services.
  • Set clear expectations, monitor outcomes, create a culture of communication, and help team members solve problems.
  • Acts on behalf of the veteran to ensure that necessary clinical services are received, and that progress is being made.
  • Tracking veterans that are consulted to the community for variety of services.
  • Completing medical form, reports, evaluation and charting as needed.
  • Educating and counselling veterans about care plans and options.
  • Coordinating and implementing of healthcare services for veterans.
  • Monitor vital signs as needed, identify changes and report abnormal findings to the PMD and treatment team.
  • Notify RLS staff via email/documentation systems of all veteran's appointments which require transportation
  • Update emergency contact information binder in front office (replace any missing documentation)
  • Coordinate medical training (CPR, Defibrillator, Naloxone etc.) for staff as required.


Requirements:

  • LPN License
  • BLS/CPR
