Position Highlights:
- Position: Mental Health Social Worker LSW
- Hourly Pay Range: $29.09 - $45.69. The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.
- Location: Chicago, IL
- Full Time - 40 hours/week
- Hours: Monday-Friday, 8:00am - 4:30pm, rotating weekends & On call
- Required Travel: No
A Brief Overview: Provides clinical social work services, along with program and policy development, and marketing. Acts in accordance with hospital and department philosophy, the NASW code of ethics, federal, state and local laws, and other regulatory bodies.
What you will do:
- Performs psychosocial assessment/evaluation of patients and their significant others.
- Implements a plan of care to attempt to resolve psychosocial problems.
- Coordinate discharge plans.
- Conducts group activities.
- Conducts individual and family meetings.
- Completes and maintains department records and documents case activities in the medical records.
- Participates in program development and Performance/Quality Improvement initiatives
- Participate as a part of a multidisciplinary treatment team in the treatment planning and discussion.
- Attends team and staff meetings, as well as other meetings as required.
- Maintains timely and complete documentation including documentation of an individual's attendance in groups, response to activities and other pertinent observations in accordance with department policy.
- Acts as intake coordinator for referrals. to the Partial Hospital Program (delete this yellow highlighted part)
- Performs Utilization review activities as needed/assigned.
- Performs other duties as assigned or requested by the nurse manager or designee.
What you will need:
- Masters Degree Social Work Required
- Recognizes needs and behaviors of various age groups. Understands human development stages and addresses age-related issues in psycho assessment and interventions.
- Licensed Social Worker (LSW) - Illinois Department of Financial and Professional Regulation (IDFPR) Required
