In-Home Youth & Family Therapist Opportunity - 24, 32, or 40 hours per week

Fuller Hospital is a 109-bed licensed, private psychiatric facility located in South Attleboro, Massachusetts providing inpatient and outpatient behavioral health services to residents of Massachusetts and Rhode Island. Inpatient units are designed to treat adults with general psychiatric or intellectual disabilities, as well as adolescents with general psychiatric issues. Patients with co-occurring psychiatric and substance abuse disorders can be treated on most units. Fuller Hospital also provides a less intensive Partial Hospitalization Program (PHP) to patients with psychiatric and co-occurring psychiatric and substance abuse disorders.

The In-Home Therapy Program is a service that provides youth with intensive therapy and family sessions in the home, guided by a master's-level clinician (IHT) and a bachelor's-level support staff (TT&S). Our goal within IHT is to meet with the family (often multiple times per week), in order to improve family dynamics, healthy functioning within the home/community, and to ultimately prevent them from stepping up to a higher level of care. We work intensively with the family until they are stabilized and able to transition to a lower level of care (i.e., outpatient therapy).

Responsibilities

  • As an In-Home Therapist (IHT), you will be facilitating intensive in-home family therapy sessions with youth under the age of 21 and their families with the goal of improving the family's ability to provide effective support for the youth to promote his/her healthy functioning within the family. This will include implementation of various therapeutic interventions, parent coaching, crisis intervention, psycho-ed., etc.
  • You will also have a bachelor's level para-professional, a Therapeutic Training & Support Staff (TT&S) assigned to your cases. The TT&S will assist you with co-facilitating sessions, treatment planning, and case management. They will work with the client and/or family to practice the skills that you teach in family sessions.
  • Completion of various paperwork such as progress notes, comprehensive and CANS assessments, safety plans, treatment plans, etc.
  • Availability for crisis intervention, crisis calls, and skills coaching as needed for your clients.
  • Frequent care coordination with collateral contacts regarding treatment plan goals and progress.
  • Identify and utilize community resources; provide aftercare referrals to develop and maintain natural supports for the youth and parent or caregiver(s) in order to sustain treatment progress.

Position Structure

Client Diagnoses: We see a variety of diagnoses and presenting problems; such as trauma, behavioral problems, depression, anxiety, autism, ADHD, ODD, etc. This provides a great deal of experience for the clinician.

Hours:

    • We are currently hiring both part time (24 Hours) and full time (32 or 40 hours) staff.
    • You create your own schedule based on you and your client's availability.
    • The typical work day hours vary as you will be making your own schedule with the families. During the school year, sessions often take place in the afternoons once school is out. However, you are welcome to schedule appointments during the day, if applicable, and on weekends if you and the family prefer to do so.
    • You are able to work from home (besides your sessions) with the laptop that is provided for documentation and zoom when applicable. You are not required to come into the office during any specific times. We do have office space and computers available should you choose to come into the office.

Caseload: Caseload depends on your status and how long/how often you are seeing your clients. If you are part-time (24 hours) you may have around 4-6 cases; full time you may have 8-10 cases. There is no set amount of clients you would have to take, just however many clients it takes to meet your hours.

Areas of Service: We service the Southeast Region of MA. The bulk of our referrals come from towns such as Attleboro, Taunton, Mansfield, Foxboro and surrounding areas. We also take cases in New Bedford/Fall River area for clinicians who live in that area. We try to keep you centralized in a specific area to avoid extensive travel. We also try to obtain referrals in your desired areas when possible.

Crisis Stabilization: Your clients may call you if they are in crisis and in need of support We create safety plans with all of our clients so they have a safety plan protocol to follow which outlines skills for them to use and supports that they can call (typically including IHT, TT&S, TM, outpatient counselor, natural supports, mobile crisis, etc.). If you are unavailable to take the crisis call, you can direct them to call our 24-hour crisis line.

Supervision: You will receive one hour per week of face-to-face supervision with a licensed clinician (in-person or via zoom). This hour can be spent discussing your cases, identifying interventions for treatment, reviewing paperwork, conceptualizing cases, or reviewing any concerns or questions you may have, etc. In addition, weekly group supervision is now offered.

Staff Meeting: Staff meetings are mandatory and are 1 hour per week on Thursdays from 12-1 pm. In staff meetings, important program related information is shared, community resources are provided, case consultations and discussions occur, as well as trainings provided. Meetings are typically held via zoom with one meeting in-person one week per month (depending on COVID numbers/situation).

Paperwork:

    • Progress notes and billing for the week are due on Monday's by 2:00 pm. You will be writing progress for each client throughout the week documenting everything being done with/for the client such as sessions, phone calls, treatment planning, etc. Progress notes/billing can be securely emailed so you do not have to come to the office to submit.
    • We provide emails with the due dates for other paperwork, such as CANS assessments and treatment plan updates which are both due every 3 months.
    • We do not have an electronic record system in place yet; however, we do provide copies of fill-in electronic versions of all paperwork. Our admin staff will file all of your paperwork into the charts.
    • As an additional convenience, we also obtain your insurance authorizations for you.

Telehealth: While navigating the current state of the pandemic, we are offering both telehealth and in-person, socially distant sessions with masks. This decision will be made between the family and the clinician. In many cases, families prefer a hybrid, where clinicians may see clients both in-person and through telehealth.

Qualifications

Requirements

  • Master's degree in a counseling or social work related field
  • If you are in your 2nd year of a master's program, you may be eligible to intern as an IHT through a paid internship
  • Licensure or licensure eligibility preferred
  • Outpatient or home based social work experience preferred
