Without them, I'd be waiting somewhere, waiting for someone to give me a chance to live..." - Client from Telecare What You Will Do to Change Lives The Family Peer Recovery Coach works to engage family members and the prospective participant with the goal of fostering a willingness to accept available services. The Family Coach provides information, education, and resources to family of participants to enable them to better support their loved one in their goals and/or recovery plan, whether it be while person is living independently or in the family home. The Family Peer Recovery Coach provides direct support and case management services to assigned family members and participants and works closely with participants and their families to ensure that their needs are expressed and that they have a "voice" in their recovery plan. In conjunction with the clinical team, the Family Peer Recovery Coach completes program intakes/assessments, facilitates groups, promotes engagement and provision of peer and family support services; and serves as an advocate for the participant and family for services as a point of interaction between participant's family and the program. Shifts Available: Full-Time | AM /DAYS | Shifts: 8:00 AM - 5:00 PM | Days: Monday - Friday Expected starting wage range is $21.00- $23.71. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements. o California: obtain Medi-Cal Peer Support Specialist Certification through CalMHSA prior to hire date and maintained throughout employment.

Telecare TAY ROC provides case management services to adults, aged 16 to 24, who utilize emergency rooms, inpatient settings, and crisis services to regularly manage their mental health and substance use needs. Through a "whatever it takes" outreach and engagement approach, these programs help reduce the need for inpatient and emergency room care while improving mental health outcomes for the members served by connecting members to supports and resources available within their community. These programs work specifically with Alameda County residents who are not yet connected to county healthcare services. Program staff will provide linkages, supports, and resources to help community members remain in the least-restrictive, most self-sufficient, and recovery-oriented settings possible. The goals of these programs include reducing inpatient and emergency room care and improving mental health outcomes in Alameda County.
