Work Schedule: 80% FTE, The scheduled shifts are Monday - Friday, between 8:00 am to 5:00 pm. No weekends, nights, or holidays. We are seeking a Registered Nurse to: Join a freshly redesigned RN Care Coordination team which supports adult patients living with chronic conditions. Connect patients with resources to help them manage their health condition(s) and achieve healthier lives. Communicate with patients/families via telephone and electronic medical record, providing both your triage and care coordination skills. Collaborate with an interdisciplinary team of physicians, advanced practice providers, social workers, schedulers and other nurses to care for patients enrolled in the program.
