Overview

The OP Care Management Assistant serves as non-clinical support to OP Care Management team members by completing administrative tasks including but, not limited to; prescription, transportation, Carle Financial Assistance Program applications, Illinois Medicaid applications, phone call management for the department, telephonic follow up with patients as requested by OP Care Management RN and/or Social Worker and work collaboratively with OP Care Management staff to connect patients with community services. Maintaining and entering patient data on new and existing patients supported by the OP Care Management team.

Responsibilities

Manage telephonic and Electronic Medical Record referrals for assistance with management of patients attributed to the Outpatient Care Management program.
Assess patient eligibility for pharmaceutical assistance programs and assist with completion and submission of the program application and supporting documents.
Facilitate assistance telephonically, to offer available resources to help patient effectively obtain their prescription medications, arrange transportation services, and complete financial assistance documentation.
Demonstrate accuracy in communication and documentation with the patient and health care team regarding all interactions related to patient care.
Demonstrate accurate documentation and communication to support encounters with patients within the organization's Electronic Medical Record. Accurately and efficiently verify patient demographics and update patient information in Electronic Medical Records.
Maintain patient schedules for upcoming appointments, verify future patient appointments and ability to validate physician schedules within the Electronic Medical Record.
Telephone Interactions: As directed, completing necessary telephone communications with patients, families, pharmacies, and/or other health care facilities. Gathering information and maintaining patient information.
Responsible for necessary correspondence with patients and care team. Ability to communicate to the OP Care Management RN any urgent needs of high risk patients that are calling seeking medical advice.
Responsible for necessary correspondence with patients and care team and any other assigned administrative duties to assist the department in care coordination efforts for patients enrolled in the Care Management program.
Communicate with the patient and health care team accurately and document accordingly. Manage referrals form Care Managment team members. Support telephonic touch points, incoming and outgoing, with patients enrolled in Care Mangement program.

Compensation and Benefits

The compensation range for this position is $15.62per hour - $25.3per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
