Summary of Need: Pharmacists will review, monitor, compound, and/or dispense, as well as maintain medications and records following prescriptions issued by an authorized prescriber. May also provide drug information and medication counseling, collect, analyze, and report on medication therapy related data, be responsible for maintaining safe and effective operations within the pharmacy environment, participate in the review of provider requests for Medicaid coverage, interpret and assist with Medicaid payment rules, regulations and policies, and assist in the development and implementation of cost containment. Perform all clinical duties associated with the profession: Review, monitor, dispense medications. Maintain medications and records pursuant to valid orders written/entered by an authorized prescriber. Provide drug information and medication counseling to patients, medical and nursing staff. Analyze and report medication therapy related data. Monitor orders for accuracy, drug allergies, rational drug therapy and incompatibilities. Familiar with the proper disposal of medications and hazardous waste Requirements: Must possess a valid degree and New York State licensure. Must be a licensed Pharmacist in NYS in good standing. Must have knowledge of medication cart and unit dose dispensing. Must have the ability to work independently, accurately and be detail oriented.
