The Senior Enrollment Specialist is a member of the Clinically Integrated Network's Ambulatory Care Management team and is accountable for patient enrollment and support of patients in the program. The Senior Enrollment Specialist works as part of a multi-disciplinary Chronic Care Management team under the direction of the Ambulatory Care Management leadership. The Senior Enrollment Specialist will serve as the point person for the complex triaging of new patient referrals and downstream communication processes. The Senior Enrollment Specialist is responsible for performing patient outreach to educate and enroll patients into the program. Once enrolled, the Senior Enrollment Specialist supports the Chronic Care Managers on patient tasks to meet the needs of the patient. EEO/AA/Disability/Veteran Responsibilities
Qualifications EDUCATION High school diploma or equivalent required. Associate or bachelor degree preferred. EXPERIENCE Two to four years of customer service experience in a patient care area of a healthcare environment. At minimum one year of successful medical call center experience required. Experience and familiarity related to symptom assessments with documentation in the electronic medical record based on call presentation preferred. SPECIAL SKILLS Excellent oral and written communication skills. PHYSICAL DEMAND Role is primarily a remote work position with the ability and expectation to travel to onsite practice locations from time to time as requested by management.
