Conduct psychosocial assessments to identify emotional, social, and environmental factors affecting pediatric patients and their families.
Develop individualized treatment and care plans in collaboration with healthcare teams and pediatrician. Advocate for families by connecting them with community resources, financial assistance programs, and specialized services. Monitor, evaluate, and document patient progress and treatment outcomes. Collaborate with healthcare providers, educators, and community organizations to ensure seamless patient care. Education: Master's Degree in Social Work or Mental Health & Behavioral Medicine (required). Licensure: MA LCSW, LICSW, or LMHC. Experience: At least 1 year of related clinical experience in a medical or pediatric setting. Knowledge of pediatric community resources and social services strongly preferred.
