Empath Health is seeking a clinical care navigator to join the team.

Monday-Friday, 8am-5pm

Primary tasks, duties, and responsibilities:

  1. Provides expert, individualized coordination of care to patients and families.
  2. Manage and coordinate all aspects of physician scheduling.
  3. Coordinate with business office for insurance verification and any documentation needs.
  4. Collaborates with treating clinicians both in the inpatient and outpatient settings to ensure appropriate, compassionate and quality care.
  5. Completes initial and on-going comprehensive physical and psychosocial assessment of the patient and family to identify needs and areas for intervention.
  6. Ensures appropriate referrals to specialists when indicated and support adherence to treatment plan, including physician visits for evaluation and on-going management.
  7. Assists patients in accessing appropriate services and resources in the community to improve their overall health and well-being.
  8. Educates patient and family to enhance understanding of disease processes and plan of care, including discussion of treatment options to ensure informed decision making.
  9. Provides emotional support and counseling to patients and their families to improve transitions, adjustment to illness and adherence to treatment plan.
  10. Evaluates the patient's physical and emotional recovery after treatment utilizing appropriate resources as needed.
  11. Empowers patients to take control of their health care by encouraging appropriate goal setting and discussions about advance care planning/development of advance directives.
  12. Builds relationships with other clinicians involved in the patients' care.
  13. Promotes patient-centered approach to care.
  14. Partners with healthcare team to manage transitions of care between hospital, primary and specialty care
  15. Facilitates proactive discharge planning or follow-up to promote better outcomes, decreased length of stay and reduced readmission rates.

Requirements:

  1. Associate Degree preferred
  2. Current and Valid LPN or MA certificate in the State of Florida
  3. CPR, BLS/ACLS Certification
  4. Minimum 4 years' experience in an acute or post-acute care setting
  5. Experience in patient education, planning, and management desired
  6. 1 year Case Management experience preferred
  7. 1 year of community health (home health, rehab, hospice, etc.) experience preferred
  8. An equivalent combination of professional experience and education may be considered as meeting the Education/Professional requirements
