CTO Social Needs Care Manager

Job Summary:

MedStar Health is looking for a CTO Social Needs Care Manager to join our team at MedStar Accountable Care.We are looking for someone with strong communication skills, the ability to effectively engage patients in a therapeutic relationship and working knowledge of differential diagnosis of common mental health and/or substance use disorders.

The CTO Social Needs Care Manager will function as a core member of the Care Transformation Organization. The Care Manager, Behavioral Health/Social Needs employs advance practice interventions to assess the interrelated social, emotional, environmental, and psychological factors that impact patient/family functioning. Initiates, implements, and evaluates interventions that are culturally sensitive and developmentally appropriate. The Care Manager is responsible for providing, coordinating, and supporting behavioral health care, social need services, and referral coordination to clinically indicated services outside of the primary care setting.

Primary Duties:

• Provides biopsychological assessments and develops treatment plans based on psychological, social, medical, and fiscal realities. Provides diagnosis and treatment employing the following modalities: problem solving, crisis intervention, decision making, bereavement counseling, individual, family and group therapy. Provides brief interventions using evidence-based techniques such as Behavioral Activation, Problem-Solving Treatment, Motivational Interviewing, or other treatments as appropriate. Provides consultation to Social Workers, Care Manager Supervisors, Care Managers, and Community Health Advocates when necessary.

• Manages risk factors of patients/families (e.g., legal, immigration status, language barriers, financial complications, employment, and disability). Documents patient progress and treatment recommendations in medical records so they can be easily shared with PCPs (Primary Care Physicians) and other treating providers. Facilitates patient engagement and follow-up care. Track patient follow-up and clinical outcomes. Documents encounters in medical record. Facilitates referrals for clinically indicated services outside the primary care clinic such as social services, housing assistance, vocational rehabilitation, mental health specialty care or substance abuse treatment.

• Monitor patients by phone or in person for changes in clinical symptoms, treatment side effects or complications.

• Participates in regularly scheduled caseload consultation with the supervisor and/or treatment team and communicates resulting treatment recommendations to the patient and the patient's primary care provider.

• Screens and assesses patients for common mental health, substance abuse disorders and social needs. Provides basic education and available treatment options. Provides referrals to resources as indicated. Supports and closely coordinates mental health care with the patient's primary care provider and, when appropriate, other mental health providers. Actively collaborates with social work colleagues to cover vacancies and provides coverage, as required, to meet departmental efforts for service improvement.

Qualifications:

• Master's degree in social work, Psychology, or related field.

• Experience working with patients who have co-occurring mental health, substance abuse and physical health problems.

• Experience with screening for common mental health and/or substance use disorders, patient assessments, and treatment planning.

• LCSW-C - Licensed Certified Social Worker - Clinical or LMSW - Licensed Master Social Worker - State Licensure or LCPC (Licensed Clinical Professional Counselor) or Licensed Grad Prof Counselor - LGPC or PhD in Psychology.
