Responsibilities Job Summary The Nutrition Educator II evaluates risk criteria, assesses nutritional status and develops/implements a nutritional plan of care for outpatients based on evidence based practice and diagnosis related care guidelines. Provide individualized nutrition assessments, counseling and nutrition recommendations based on patients' specific therapies and nutrition needs. Evaluates patient outcome and progress toward achieving nutrition goals. Revises intervention and/or education plan. Prepares outcome oriented documentation (written or transcribed) for the patient's medical record and to the referring physician on all consultations in accordance with evidence based practice guidelines Researches, develops and implements patient nutrition care protocols and education materials, based on evidence based practice. Serves as nutrition specialist and resource to physicians, departments, allied health professionals, and accredited nutrition programs. Participates in performance integration and improvement of patient care processes and department or system process improvement initiatives. Adapts to changes in work environment. Recognizes and performs duties and responsibilities which need to be done although not specifically assigned. Attend education programs to maintain status as effective and knowledgeable resource regarding patients' nutrition needs. Qualifications Education Bachelor's Degree Required Experience 2 years of acute care in-patient clinical nutrition experience Required Licenses and Certifications Must be a Registered Dietitian under CDR (Commission for Registered Dietitian) Upon Hire Required
