Responsibilities

Job Summary
The Nutrition Educator II evaluates risk criteria, assesses nutritional status and develops/implements a nutritional plan of care for outpatients based on evidence based practice and diagnosis related care guidelines.
  • Provide individualized nutrition assessments, counseling and nutrition recommendations based on patients' specific therapies and nutrition needs.
  • Evaluates patient outcome and progress toward achieving nutrition goals. Revises intervention and/or education plan.
  • Prepares outcome oriented documentation (written or transcribed) for the patient's medical record and to the referring physician on all consultations in accordance with evidence based practice guidelines
  • Researches, develops and implements patient nutrition care protocols and education materials, based on evidence based practice.
  • Serves as nutrition specialist and resource to physicians, departments, allied health professionals, and accredited nutrition programs.
  • Participates in performance integration and improvement of patient care processes and department or system process improvement initiatives.
  • Adapts to changes in work environment. Recognizes and performs duties and responsibilities which need to be done although not specifically assigned.
  • Attend education programs to maintain status as effective and knowledgeable resource regarding patients' nutrition needs.

Education
Bachelor's Degree Required

Experience
2 years of acute care in-patient clinical nutrition experience Required

Licenses and Certifications
Must be a Registered Dietitian under CDR (Commission for Registered Dietitian) Upon Hire Required
