As the Community Health Worker (CHW) Supervisor, you will be a vital member of the Homeless Healthcare Collaborative (HHC) team, working to improve health equity and clinical outcomes for people experiencing homelessness across Los Angeles County. In this leadership role, you will provide direct supervision to CHWs while fostering collaboration between medical, behavioral health, and case management services delivered in the community. Primary duties include: * Supervise and provide leadership to the CHWs within Homeless Healthcare Collaborative team. * Provide daily administrative leadership to support operational effectiveness, personnel management, regulatory compliance, and performance improvement. * Act as a liaison between CHWs, clinical teams, patients, caregivers, physicians, community partners, and office staff to strengthen partnerships and enhance patient care. * Travel to community sites as needed to support CHWs, ensuring they have the necessary resources to succeed. * Promote staff education, enforce policy implementation, and oversee safety and risk management efforts. * Develop and facilitate trainings for CHWs. * Assist in driving HHC mobile clinics/University vehicle as needed. Salary Range: $68,800.00 - $141,000.00/year. Qualifications
* Minimum 4 years of relevant experience in patient navigation, case management, community health work, or homeless services. * Prior experience working with people experiencing homelessness. * BLS from the American Heart Association or American Red Cross. * Required to hold valid driver's license, have a driving record that is in accordance with local policies/procedures, and/or enroll in the California Employer Pull Notice Program. * Ability to interact diplomatically and empathetically with patients in varying degrees of health and effectively communicate with staff, faculty, and the general public. * Computer skills in EMR, Word, Excel, PowerPoint, and Outlook. * Prior supervisory experience - preferred. * Experience with Whole-Person Care or Enhanced Care Management (ECM) work - preferred. * Experience in street medicine or community-based care for people experiencing homelessness - preferred. * Lived experience of homelessness - preferred. * Bilingual proficiency in English and Spanish - preferred. |
