Responsibilities Job Summary The Clinical Services Liaison - RN is responsible for: Positive and effective representation of HonorHealth Home Infusion and introducing the full scope of services offered to hospital case managers, physicians, nursing agencies and other referral sources. Ensuring ongoing updates and education for referral sources. Responsible for the successful clinical coordination of care from the referral source into the home setting on behalf of HonorHealth Home Infusion. Provides and utilizes clinical expertise and skills to assist referral sources &/or patients with: Identification of appropriate candidates for home infusion &/or home health care Identification of the patients' and/or caregivers' ability to provide care Patient education and training regarding therapy and supplies, as well as, clarifying expectations of care in the home. Medication connection at the hospital / facility to expedite the discharge into the home setting, as appropriate. Coordinates all aspects of home infusion initiation and ongoing monitoring as required
Qualifications Education Associate's Degree of Nursing from accredited college or school of nursing Required Experience 2 years in case management or home health Required Licenses and Certifications Registered Nurse (RN) State And/Or Compact State Licensure Required |
