Responsibilities

Hours for this role are Monday - Friday, 8a-430p

What You'll Do: As a Social Worker in our IOP/PHP, you will be at the forefront of patient care, providing essential services that include:

  • Facilitating Admissions: Welcome patients into our programs, ensuring a smooth transition and understanding of the treatment process.
  • Leading Psychoeducation Groups: Conduct engaging and informative psychoeducation groups that empower patients with knowledge about their diagnoses and treatment options.
  • Comprehensive Assessments: Perform effective psychosocial assessments and suicide risk evaluations to inform treatment planning and ensure patient safety.
  • Collaborative Treatment Planning: Develop and revise treatment and complex discharge plans, communicating these effectively with patients, their support systems, and the treatment team.
  • Patient Advocacy: Act as a liaison between patients and their support systems, keeping them involved in treatment plan decisions and fostering open communication.
  • Psychoeducation: Provide valuable psychoeducation to patients and their families on various topics, including symptom management, safety plans, patient rights, and financial considerations.
  • Ethical Care: Deliver patient-centered, culturally appropriate, and age-appropriate care, advocating for the best interests of each individual.
  • Maintaining Competency: Stay current with effective de-escalation techniques, therapeutic self-care, relationship-based care, and trauma-informed practices to ensure a safe and supportive environment.

Qualifications

  • Bachelor's Degree in Social Work required, Master's Degree in Social Work preferred, with licensure in the State of Minnesota required.
  • Experience in a mental health setting preferred Master's Degree in Social Work preferred, with licensure in the State of Minnesota required.
  • Experience using electronic health records preferred
  • Proficient in Microsoft Office Products including Word Outlook, and Teams
  • Able to effectively and efficiently communicate in written and verbal form
  • Ability to smoothly and rapidly shift between dissimilar tasks
  • Ability to communicate well both verbally and in writing
  • Ability to respond appropriately in times of mental health crisis

Compensation for this role ranges from $54,080 to $75,795 depending on education and experience.

An individual's pay rate within the posted range is determined by several factors, including skills, experience, education, and market conditions. Additionally, we consider internal equity to ensure fairness within our organization.
