We are searching for a talented Advanced Practitioner for Southcoast Health at Home - Palliative Consulting

Hours: 40 hrs 8:00A - 4:30P

Shift: Day - no weekend, holiday or call.

Location: New Bedford, MA to Fall River, MA community & some SNF.

Responsibilities

  • Under direction of the VNA President & CEO or designee, and in collaboration with the Physician, provide an advanced level of nursing practice to deliver palliative and related health care services to patients and families.
  • Perform a wide variety of duties including but not limited to bedside palliative care patient/family goals and care evaluation and treatment and management of serious health conditions.
  • Serve as a clinical resource and consultant to members of the interdisciplinary team.


Qualifications

  • Masters of Science Degree in Nursing is required. Certification from a National accrediting body to practice as a Nurse Practitioner is required. Current Massachusetts/Rhode Island RN license is required.
  • Minimum of 5 years nursing experience with clinical nursing care in oncology, hospice, geriatrics and/or intensive care preferred.
  • A minimum of one year Nurse Practitioner experience.
  • Certification in Hospice and Palliative Care preferred.
  • Must possess current CPR certification.
  • Certification from a national accrediting body to practice as a nurse practitioner is required. Current NP license in Massachusetts & Rhode Island is required.
  • Certification in Hospice and Palliative Care required.
  • Must possess current CPR certification.
