Nurse Navigator - Neurology

Full Time - 40 hours/week. Location will be split between 3 days at Henry Ford West Bloomfield and2 days at Detroit Main Campus Hospitals

Candidate Preferences: Neurology experience, experience working with Alzheimer's Dementia patients, working with the elderly population, ability to work in a patient and empathetic manner. Great communication skills, attention to detail, experience working in EPIC, scheduling experience, etc. Previous Nurse Navigator or Case Management experience a plus.

"About the position

This position involves acting as the lead communicator and overseeing the infusion patients' care team, ensuring that patients and caregivers are well-informed and supported throughout the treatment pathway. The Nurse Navigator will be responsible for managing logistics related to infusion and MRI scheduling, as well as coordinating post-infusion follow-ups, which may include virtual visits to assess any reactions and address patient inquiries. This role requires a proactive approach in providing resources to patients and families, connecting them with local organizations that offer care and support, and collaborating closely with the supervising physician to address any potential issues that may arise during treatment. In addition to patient coordination, the Nurse Navigator will perform advanced administrative tasks such as obtaining insurance authorizations, triaging calls, and responding to complex inquiries from stakeholders. The Nurse Navigator will serve as the primary source of communication for patients regarding infusion/testing expectations, treatment plans, and follow-up metrics. This position also involves coordinating pre-infusion workups, infusion scheduling, and post-infusion appointments, ensuring that all necessary appointments and procedures are aligned with the patient's care plan. The role requires maintaining accurate databases in accordance with therapy protocols, and assisting in the preparation of meetings and reports. The Nurse Navigator will also be expected to demonstrate proficiency in various software applications to effectively document and manage patient information, as well as prepare presentations and reports as needed.

Responsibilities

* Act as the lead communicator for Alzheimer's Dementia infusion patients and oversee their care team.



  • Coordinate logistics for infusion and MRI scheduling along the treatment pathway.
  • Perform post-infusion follow-up calls to assess reactions and address patient questions.
  • Obtain insurance authorizations for advanced imaging and treatment protocols.
  • Respond to complex inquiries and assist stakeholders with care coordination.
  • Maintain required databases per therapy plan protocol and update as necessary.
  • Assist in the preparation and coordination of meetings, interviews, and conferences.
  • Prepare presentations, letters, reports, and related materials.
  • Manage office maintenance and supplies, placing orders as needed.


GENERAL SUMMARY: Nurse Navigators are nurses with specialty-specific clinical knowledge who offer individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers across the continuum of care. Navigators Collaborate with other health care team members including physicians, nurses, advanced practice providers, social workers, financial counselors, community health workers, patient access coordinators, and ancillary and other support staff, and referring physicians to create a high-quality, efficient, safe, and exceptional patient experience.

EDUCATION/EXPERIENCE REQUIRED:

Graduate from an accredited school of nursing required. Bachelor's Degree preferred.

Two (2) years of nursing experience required. Specialty experience preferred.

Maintains and demonstrates current clinical knowledge in area of specialty.

Strong organizational, leadership, listening, critical thinking, problem solving, and interpersonal skills.

Strong communication skills, written and verbal.

Ability to prioritize and reprioritize quickly.

Ability to develop collaborative relationships both internally and externally.

Ability to work in teams and in a self-directed, autonomous manner. Able to delegate and coordinate efforts of the team/co-workers.

Compassionate and empathetic.

Understands Musculoskeletal Health specialties, pathways and procedures, goals of care, symptom management, quality of life, and evidence-based practice guidelines.

Demonstrates excellent coaching and mentoring skills.

Takes necessary actions to develop and maintain personal job competencies/expertise and to improve performance on an ongoing basis; keeps current regarding medical and technological trends in healthcare overall and/or specialty/service line best practices, as well as a working knowledge of insurance rules and community resources.

Computer skills and knowledge of Windows, Outlook and electronic medical record.

CERTIFICATIONS/LICENSURES REQUIRED:

Current licensure by the Michigan State Board of Nursing as a Registered Nurse.

Specialty-specific certification preferred and desired within two (2) years of hire as appropriate to job being filled.

NOTE: The Nurse Navigators for the Musculoskeletal Health Service Line already employed by HFHS orthopedic services have two (2) years to complete certification after entry into this job.
