Position on this function is under the direction of the RN Manager/Supervisor, the Social Worker will collaborate with the Cardiology Providers and the entire clinical team to provide quality and appropriate patient care, with utilization of excellent customer service skills throughout the patient care continuum.

Position on this function is responsible for coordinating social services for all Southwest Medical Cardiology patients and those patients that are at high risk for hospital admission. Collaborates with the Cardiology Providers, RN Case Manager and the entire care team; to address the entirety of the patient's needs and to promote better patient outcomes. Responsible for managing/review of referrals, completing in-person and/or telephonic patient assessments, follow up with the availability of a home visit if needed and resource navigation related to Social Determinants of Health. Provide great customer service to ensure a quality patient experience through fostering a caring culture that demonstrates excellent customer service, while achieving SMA/Department goals. Develops individualized care plans that optimize the psychosocial and financial factors impacting transitions of care. Acts as a resource for the patient, their families and the health care delivery team in the outpatient setting, identifying and managing referrals to community resources. Ensures appropriate standards of care are implemented and in compliance with all state and federal regulations, standards of practice, corporate policies and procedures.

Primary Responsibilities:



  • Foster a workplace culture that is consistent with the overall organization and emphasizes the identified mission, vision, guiding principles, and values of the organization
  • Provide patient care (direct and indirect) with exemplary customer service to all SMA Cardiology patients
  • Assist with the coordination of patient care under the guidance of the Cardiology Providers, acting as a liaison in person or by telephone for social services
  • Identify, Assist, Coordinate and initiate referrals for social service programs including financial, psychosocial, community and state supportive services
  • Acts as an advocate and resource for patients, their families and the health care delivery team, in the outpatient setting, for social services
  • Manage a clinical caseload, organizing and prioritizing the workload effectively, with regular supervision and guidance
  • Duties include but not limited to formulation and management social service plan of care throughout the continuum of care as a single point of contact for outpatient social services; documents all patient assessments and services provided in the electronic medical record including referrals to social service programs and follow-up documentation to ensure referral was received by the social services agency
  • Participate in team meetings, data management and measurement of quality improvement indicators of patient outcomes as is dictated by designated management/department team
  • Ability to work as part of a multi-disciplinary team. Fosters teamwork by initiating and building communication with all levels of management and employees to promote cohesiveness and improved outcomes
  • Responsible to contact patient and/or caregiver to initiate plan of care within three business days for routine referrals: two business days for ASAP referrals and one business day for Urgent referrals
  • Subject matter expert to include but not limited to community resourcing
  • Maintain, facilitate and promote department protocols, processes, policies and standards to include but not limited to the following: New and Existing patient management and other Specialty specific protocols as signed by department head or designee
  • Other duties as assigned

Required Qualifications:



  • High School diploma and/or equivalent
  • Master's degree in Social Work
  • Unrestricted State of Nevada Social Work license or eligibility for immediate provisional license
  • 1+ years of related social work experience in an acute care setting, clinic, home health or case management setting
  • Knowledge of Social Work theory/practice and ability to utilize a variety of diagnostic tools/techniques
  • Knowledge of State and community based programs, including rules and regulations applicable to these programs
  • Knowledge of Advanced directives, POLST and legal regulations, including but not limited to reporting of abuse
  • Basic computer skills to include Word, Excel, and Windows based applications and worked within Electronic Medical Record


Preferred Qualifications:



  • Understanding of managed care
  • Proven good communication skills, excellent phone etiquette and customer service skills
  • Proven excellent self-starter
  • Proven ability to perform all essential job functions with or without reasonable accommodation
  • Demonstrated ability to build and maintain community resource database


Working Conditions:



  • Normal clinic environment
  • Possess personal transportation to travel as needed throughout the Las Vegas Valley
  • Valid Nevada driver's license, auto insurance and a motor vehicle in good working condition required
  • Ability to pass FIT testing and wear protective mask as deemed by OptumCare NV in the course of work at a clinic

The salary range for this role is $59,500 to $116,600 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
