The Opportunity:

The Field Operations RN Manager is a nurse with extensive experience in complex care or case management within High Risk Care (HRC) Programs. This individual has the ability to provide sound nursing leadership within their market and nationally. They will serve as a key member of the team to extend and enhance the expertise and impact of nurses. This leader will spend approximately 30% of their time on leadership activities and 70% on direct clinical care.

Field Operations RN Manager, Responsibilities:


  • Accountable leader and manager for the nursing team across the national One Medical @ Home program (OM@H) and for High Risk Field Operations more broadly.
  • Provision of clinical guidance and support for market Nurses and nursing culture within and across the continuum of One Medical at Home and High Risk Care Programs
  • Develop and implement nursing standards of practice such as documentation and clinical pathways to support chronic disease management and focused care management interventions
  • Recruit, hire, onboard new Nurses and other team members when needed.
  • Ensure the nursing team is meeting the program process and outcome measures and goals.
  • Close collaboration with HRC leadership to ensure consistent execution of HRC's vision and model of care to market teams
  • Championship of Nursing & organizational priorities

    • Dual responsibilities representing the perspective of nurses as well as ensuring nurses navigate and embrace national / market level change and initiative
    • Accountable for RN team performance against key priorities monitored on Weekly and Monthly Business Reviews.


  • Complex Case Management - ensure the program meets expectations of delegated programs as outlined by the Population Health Program manual.
  • Clinical Care

    • Ensure clinical care standards are met by the nursing team on a daily basis.
    • Provide direct clinical care for patient panels based on staffing support needs to support overall care goals within the High Risk programs.




Qualifications:


  • Active RN License in AZ and/or WA and willingness to obtain other licenses or compact license
  • At least 3 years of experience as a RN leader in a High Risk Care program or similar programs.
  • A goal oriented, high energy, passionate perspective, with a focus on living organizational values, and ability to set the tone for a positive work culture.
  • Demonstrates outstanding clinical aptitude, emotional intelligence and critical thinking under pressure, using sound judgment in caring for patient needs.
  • Comfortable operating in ambiguity and uses flexibility and creativity to address challenges
  • Comfortable with using data to support clinical work and drive improved patient outcomes.
  • Direct management experience of at least 2 years preferred


The Field Operations RN Manager will report to the Field Operations Medical Director. They will work in close collaboration with all High Risk Care Program and Market Leaders. Example job duties included by not limited to:



  • Trains in each OM@H Market Program: Standard of Work review and understanding of market need
  • Works with Field Operations Medical Director and Regional Medical Directors to understand and set market nursing priorities
  • Maintains schedules for RN triage, PTO and other clinical duties
  • Develops and ensures standardized nursing documentation for chronic disease education and care coordination/outreach
  • Dedicated admin time for role responsibilities
  • Market support and clinical case load responsibilities
  • Leads RN team through EMR migration
  • Develops Standard Work around OM@H post-discharge process
  • Manages clinical quality and effectiveness through RN audit
  • Participates in Leadership meetings
  • Championship and coordination of local nursing participation in national initiatives
  • Represent both leadership and nursing interests bi-directionally
  • Participate in RN professional development
  • Serves as a resource for service development on Nursing specific processes
  • Participates in RN hiring process
  • A key training resource in onboarding new nurses
  • A key training resource in investing in existing nurse skills
  • Holds team accountable to overall team goals and expectations.
  • Develops clinical pathways to support chronic disease education
  • Develops education and training around goals of care
  • Other goals and duties as assigned



One Medical is committed to fair and equitable compensation practices.

The base salary range for this role is $104,000 to $111,000 based on a full-time schedule. Final determination of starting pay may vary based on factors such as practice experience and patient care schedule. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements.
