Summary Join a dynamic healthcare team where your expertise as a Social Worker makes a real difference. As a Social Worker II, you'll play a vital role in supporting patients and families through some of life's most critical moments. Working collaboratively with physicians, nurses, and interdisciplinary team members, you'll assess psychosocial needs, address barriers to care, and guide families through the healthcare journey with compassion, advocacy, and expert guidance. Your work will directly impact discharge planning, crisis intervention, family education, and the overall patient experience - helping ensure the best possible outcomes for every individual in your care. This candidate will work solely on a multi-disciplinary team devoted to patients in the General Adult Outpatient Program within the Community Psychiatry Program (CPP). The candidate provides comprehensive psychosocial assessments, diagnostic formulations, and treatment (individual, family, and group psychotherapy) for patients, provides crisis intervention when appropriate, maintains clinical documentation in electronic record, and works in conjunction with CPP psychiatrists to integrate care provided. Supports CPP initiative to implement routine outcome measures for feedback informed care, and may at times obtain insurance authorizations for services. Key Responsibilities

Required Credentials
Salary Range: Minimum $31.92/hour - Maximum $52.69/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
