Department and Program Summary

The MaineHealth Preble Street Learning Collaborative (MH PSLC) is a low-barrier clinic that addresses the needs and care of people experiencing homelessness through education, outreach, and walk-in medical services, case management, and other community resources. In 2024, the MH PSLC launched the CONNECT Outreach Van, a grant-funded mobile health and harm reduction program that delivers trauma-informed treatment for psychiatric and substance use disorders to people with unstable housing in Greater Portland.

The Harm Reduction Program Coordinator position will support growth and expansion of the CONNECT program and offers the opportunity to join a close and supportive interprofessional team that provides medical care, social work, and basic support for patients with unstable housing.

Helpful Professional Experience

Prior work experience in the following areas/topics would be beneficial for this position:

* Training in and/or experience with Harm Reduction

* Training in and/or experience with Trauma-Informed Care

* Experience working in low-barrier settings

* Experience working with people experiencing homelessness

* Background in project coordination and management

* Background in social work or mental health

* Knowledge of substance use disorders

Scope of Work

The following duties reflect the position's essential functions but do not restrict the assignment of additional tasks.

* Support day-to-day program operations of the CONNECT Outreach Van through administrative activities, community engagement, and direct patient support.

* Manage procurement, re-stocking, and inventory tracking of essential supplies for patients; this includes managing supply needs for the CONNECT Outreach Van

* Manage Harm Reduction program activities, such as inventory and assembly of harm reduction kits and creation of harm reduction communication materials.

* Manage patient referrals documentation in EPIC, as well as coordination and communication of referrals among program partners.

* Support grant and programmatic data collection and reporting activities.

* Support other tasks as assigned by the Program or Clinical Director, or as the needs of the grant change.

    Required Minimum Knowledge, Skills, and Abilities (KSAs)

    1. Education: Associates or Bachelor's Degree preferred.
    2. License/Certifications:Driver's license is required.
    3. Experience: One year of experience in healthcare or related work experience required. Some roles may require experience in a specific program area.
    4. Additional Skills/Requirements Required: Strong computer skills.
    5. Additional Skills/Requirements Preferred: N/A
