JOB DESCRIPTION:

SUMMARY OF POSITION

Have Case Management and supervisory experience? Come join the Centerstone team as a Team Lead. You'll provide direct administrative supervision to a case management and Health Link team, conduct monthly chart audits, review documentation, hire and train new staff. Through you work, you teach staff the importance of providing quality coaching and coordination of health and wellness, strong documentation skills, navigating and teaching staff to navigate electronic health record. We are looking for people who are ready to lead a team of Case Managers to make a difference in the lives of people we serve every day. The mental health needs in our community have never been greater. If you have a Master's degree in counseling, psychology, social work or related field ( or Bachelors with supervisory experience)- and you are passionate about making a difference in the lives of people coping with mental health issues, we need you!

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Maintain necessary communications and reporting with referral sources and other involved parties as approved by client.
  • Maintain effective working relationships with the clinical team
  • Maintain timely/accurate clinical records in accordance with organization policies
  • Maintain effective relationships with Community referral sources
  • Participate in the development and implementation of quality clinical practices
  • Compliance requirement for annual completion of KPI forms
  • Responds promptly to client's needs to coordination of services

KNOWLEDGE, SKILLS & ABILITIES

  • Effectively communicate via written, verbal, in person and virtual methods.

  • Work well with a team , but also be self-motivated
  • Effective at managing priorities/deadlines

QUALIFICATIONS

Education Level

Master's level degree in counseling, psychology, social work, or related field required. Bachelor's degree in psychology, social work, or related field with two years' case management and one year of supervisory experience may be accepted in lieu of a master's degree.

Years of Experience

One year case management and supervisory experience required.

$46,000.00--$64,470.00
