As a Hybrid Service Coordinator, you will provide professional support to individuals with disabilities and older Minnesotans who choose PICS as their service provider. This position requires regular interaction with a variety of people, including persons served and their families, Lead Agency staff, other provider agencies, internal PICS' staff and key LSS Central Service partners. Service Coordinators provide support to persons served and their families to help them understand and utilize their services appropriately. Service Coordinators work with persons served and their families by providing expertise in program processes, completing budget renewals and addendums, and managing the overall service delivery process.

Job Requirements:

* Strong desire to support others and make a difference through customer service and interpersonal connection.
* A four-year degree with social services emphasis is preferred.
* Experience working with people with disabilities and their families as well as experience in home and community-based service program coordination/management is preferred.
* Ability to become subject-matter expert in their specific service program.
* Knowledge of resources and support available to people with disabilities and older Minnesotans is preferred.
* Strong computer skills and ability to learn new software.
* Strong analytical skills and proficiency working with numbers.
* Ability to perform a variety of on-going tasks in a fast-paced environment.
* Ability to read, analyze, and interpret Department of Human Services' policies and regulations as well as applicable state and federal laws related to service delivery of program guidelines.
* Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the community.
* Ability to maintain consistent documentation regarding correspondence, phone and email communications.
* Must be well organized and detail oriented to complete work accurately and efficiently to meet deadlines.
