Salary: Union $18.40 with a 6 month and 1 year increase Position Details: Under the direction of the Blended Case Management (BCM) Supervisor, this position will be responsible for developing, coordinating and implementing services for consumers as identified in the service plan. This position will be based home but will have 12-15 youth and young adults (15-26 years of age) per week Case load will be located in Cambria County (mileage will be paid) As a member of the BCM team this position will serve as primary contact for youth and young adults. Core responsibilities will include providing case-management, service planning and coordination of services with other agency providers to meet the clients' needs along with supportive counseling.
