GENERAL SUMMARY:

Responsible for accurate and timely chart preparation for all Bryan Heart patient appointments. Develops and maintains effective and professional communication (verbal and written) with inner and outside organization professionals facilitating the needs of the department.

PRINCIPAL JOB FUNCTIONS:

1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values.

2. *Works with scheduling department to ensure clinic appointment schedules are current and accurate regarding patient information.

3. *Prepares the patient electronic health record for upcoming appointments by scanning and indexing received medical documentation to assist the clinical team in providing medical care.

4. Collaborates with clinical support staff to address clinical documentation gaps and ensuring a positive patient/provider experience.

5. Develops and fosters positive relationships with local and out of state primary care offices/referring offices/facilities/etc. to obtain appropriate and necessary patient medical documentation.

6. *Professionally communicates with patients to obtain additional medical information assisting in the composition of an accurate and complete patient medical record as needed. Obtains necessary release of information authorizations.

7. Scans outside correspondence (office notes/letters/non solicited results/etc) and indexes to accurate location in the patient medical record.

8. *Receives and processes outside provider/facility/insurance payer requests required for continuity of care or payment of healthcare operations

9. Scans and releases orders as needed to result orders placed by providers. Assists in open note/encounter, missing signature, missing order deficiency tracking, reconciliation and resolution process.

10. Effectively communicates department concerns, needs and desired resources (technological/staffing) to leadership.

11. Actively participates in meetings to build a cohesive team by responding to inquiries, making recommendations, sharing observations and respecting team members input.

12. Participates and completes mandatory annual training as assigned by Bryan Heart/Bryan Health.

13. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.

14. Participates in meetings, committees and department projects as assigned.

EDUCATION AND EXPERIENCE:

High School diploma or equivalency required. Minimum of one (1) year office experience with focus on medical records processes and procedures preferred.
