As a Primary Care Provider on this innovative team, you'll deliver trauma-informed, person-centered care from a fully equipped RV and on foot, reaching individuals where they are in encampments, at community events, and along the streets of Portland.
What you'll do:
- Provide street-based primary care, addiction medicine, and mental health support to unsheltered individuals.
- Carry a backpack full of essential medical supplies and deliver care with empathy, flexibility, and expertise.
- Support Hepatitis C treatment, low-barrier buprenorphine access, and urgent care needs in real time.
- Collaborate with a tight-knit, interdisciplinary team focused on health, resiliency, and engagement.
- Participate in quality improvement, data collection, and mobile care innovation.

Salary Ranges:
Nurse Practitioner $108,000 - $140,000
Physician Assistant $108,000 - $140,000
Naturopath $113,000 - $147,000
Physician $185,000 - $240,000
(Salary Ranges are based on licensure/certification, professional experience, and CCC internal equity)
Minimum Qualification for all applicants:
- Demonstrated skills in primary care medicine with a minimum of 2 years' experience preferred.
- Must meet Training requirements for Buprenorphine prescribing
- Experience with Hepatitis C primary care-based treatment preferred, or provider will obtain training within 30 days of employment
- Must have current BLS certification
- Must have training in de-escalation, or ability to complete within 30 days of hire
- Must have training in trauma-informed care, or ability to complete within 30 days of hire
- Current DEA Certification required or ability to obtain within 30 days of employment.
- Must possess a current driver's license, pass a DMV background check, and be designated an "acceptable" driver as outlined in Central City Concern's Fleet Safety Policy. Must pass an initial driver training and an annual recertification training.
- Must be able to carry 30lbs.
- Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit.
- Must meet CCC privileging requirements as required by FTCA.
- Spanish language fluency preferred.
- Wound care experience highly preferred.
MINIMUM QUALIFICATIONS: PHYSICIAN (MD/DO)
- Board eligibility or certification in Internal Medicine or Family Practice.
- Licensed to practice as a Physician in Oregon.
MINIMUM QUALIFICATIONS: NURSE PRACTITIONER
- Graduate of an accredited Nurse Practitioner Program and unrestricted licensure to practice medicine in Oregon or eligibility for such licensure.
- Appropriate board certification.
MINIMUM QUALIFICATIONS: PHYSICIAN ASSISTANT
- Graduate of an accredited Physician Assistant Program and unrestricted licensure to practice medicine in Oregon or eligibility for such licensure.
- Board certification with NCCPA.
MINIMUM QUALIFICATIONS: NATUROPATH
- Graduation from a CNME-accredited, four-year Naturopathic Medical program.
- Must have completed a CNME-certified primary care residency program.
- Licensed naturopathic physician in the State of Oregon.
MINIMUM QUALIFICATIONS: PMHNP
- Bachelor's degree in nursing and Master's degree in Psychiatric/Mental Health Nursing from State Board of Nursing approved schools of nursing.
- Current license to practice nursing in Oregon and certified as a PMHNP by the Oregon State Board of Nursing and National certification.
Responsibilities:
- Work as a member of the Mobile Health team to deliver comprehensive, client-centered, and trauma-informed services.
- Proactively identify opportunities for improvement within Mobile health workflows and logistics. Participate in review and collection of Mobile Medical Key Performance Indicators; review and contribute to continuous improvement opportunities and plans.
- Support patient success in achieving Health, Resiliency, and Engagement.
- Determine and formulate health histories and perform physical exams.
- Support pharmacy-based Hepatitis C program and offer medical visits to support clients receiving treatment.
- Determine and manage minor and/or acute illnesses and initiate treatment.
- Prescribe medications, perform procedures, and other forms of treatment as necessary.
- Keep appropriate records within the electronic health record.
- Participate in and, at times, lead Provider Education Sessions on a variety of clinical topics, including helping train and onboard other Mobile Medical providers and staff.
- Participate in compliance, quality assurance, and quality improvement activities as directed.
- Collaborate with the care team to ensure optimal performance on team- and mobile based performance metrics.
- Attend all mandatory CCC trainings in a timely manner.
- Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
- Perform other duties as assigned, including occasional work at other CCC clinical sites and/or off-site at community events.

- Ability to walk distances
- Knowledge of de-escalation methods or willingness to be trained in these methods.
- Comfort working in an independent clinical practice with on and off-site support from medical and administrative supervisors.
