SUMMARY:

We are currently seeking a Rehab Discharge Coordinator to join our team on the Knapp Inpatient Rehab team in partnership with the Shirley Ryan Ability Lab. This full-time role will work days.

Purpose of this position: The Rehab Discharge Coordinator is an essential part of the planning, organizing, coordinating, implementing, and monitoring the services being provided during the patient's rehabilitation stay. This role aims to interview patients and caregivers on ongoing needs recommended by the multidisciplinary team to achieve discharge goals and achieving community reentry.

RESPONSIBILITIES:



  • Performs admission assessment on patient and caregivers anticipating needs to help make plans and goals for reentry to a community setting\
  • Sets up family conference to align patients, caregivers, and multidisciplinary team
  • Sets up adolescent school conferences to help with school reentry
  • Communicates with patients and caregivers disseminates pertinent dynamics to the team
  • Assists with follow up assessment interview results, assists with analyzing feedback and assists with developing an action plan
  • Provides education, guidance, and referrals to the patient on alternatives of care and services for alternatives of care within the community as well as alternative level of care
  • Coordinates the discharge Durable Medical Equipment (DME) with the multidisciplinary team
  • Coordinates with community agencies, providers, and legal representatives to ensure continuity of the individual's care
  • Enters care scores on admission and discharge in the SW FIM flowsheet
  • Assists with conducting after discharge calls to patients to evaluate improvement since discharge and request satisfaction feedback
  • Performs other duties as assigned

QUALIFICATIONS:

Minimum Qualifications:



  • Associates degree in a health-related field
  • Two years' experience in acute rehabilitation care including Electronic Health Record (EHR) navigation experience

  • An approved equivalent combination of education and experience


Preferred Qualifications:



  • Certification in rehabilitation nursing, social work, or a related specialty is highly recommended


Knowledge/ Skills/ Abilities:



  • Knowledge of resources available regarding the regulations and parameters of third-party reimbursement
  • Ability to communicate effectively in both oral and written form
  • Ability to exercise sound and independent judgment
  • Knowledge and skill in use of job appropriate technology and software applications


License/Certifications:



  • Current Minnesota licensure required: Registered Nurse, Social Work, Occupational Therapist, Physical Therapist, or Speech Language Pathologist
