Work Schedule:

60% FTE, salaried position, day/evening shift, every other weekend, and holiday rotation required. Hours are based off the operational needs of the department. This position is hybrid with onsite work and travel required within the Madison, WI area. Travel is required to University Hospital and East Madison Hospital to enroll patients into the program. Travel to patient's homes may also be required (within a 30 mile radius of University Hospital). Your assigned work location will be 301 S. Westfield Rd., Madison, WI and travel may be required to this location.

Pay:



  • Pay range is based on a full-time (100%) position.
  • Salary is prorated based on FTE.

Exciting opportunity to join our Home Based Hospital Care team providing hospital level care in the comfort of the patient's home!

We are seeking a Registered Nurse (RN):



  • Work directly with patients and families at University Hospital and East Madison Hospital to enroll them in the Home Based Hospital Care Program (HBHC) where HBHC staff utilizing care plans, interventions and equipment that are modeled after an acute care inpatient unit setting.
  • Coordinate the in-home schedule for HBHC patients to meet their needs.
  • All RN Program Coordinators are cross trained to provide hands-on acute care (as operationally needed) in the home setting for the Home-Based Hospital Care patient population requiring the use of advanced assessment, technical, teaching, and decision-making skills.
  • Assess post-acute needs and ensure that appropriate referrals to other services are made.
  • Interdisciplinary conferencing is a key component of this role's responsibility, and thorough and timely documentation in EPIC is required.

Qualifications

  • Master's Degree in Nursing, Business Administration, Management, Public Health, or related field Preferred

Work Experience

  • 2 years of work experience as a Registered Nurse Required
  • Experience in care coordination, triage, or nursing leadership Preferred
  • Experience in program development Preferred
  • Knowledge and experience in development, implementation, and evaluation of clinical programs Preferred

Licenses & Certifications

  • Licensed as a Registered Nurse (RN) in the State of WI or holds a license issued by a jurisdiction that has adopted the nurse licensure compact Upon Hire Required
  • Current CPR/BLS certification Upon Hire Required
