We are pleased to present the following position for your consideration:

Position Overview: Assessment Clinician - Remote

This full-time remote position is now available providing telehealth assessment and treatment planning for adults participating in mental health services in our Outpatient Mental Health Department. Candidates will be energetic and internally driven to assist individuals in gaining linkage to necessary services to assist in recovery of mental health and/or substance use disorders.

Clinical Responsibilities Include:

  • Telephone and video contact with clients seeking services
  • Provides case management to refer and link individuals to community resources
  • Provides engagement to clients who may need additional support to re-engage in treatment
  • Completes written assessments, treatment plans, and progress notes within established timelines
  • Coordination and active participation in the referral and linkage healthcare providers
  • Participate in a team-based approach to providing treatment within the outpatient team
  • Will attend weekly staff meeting with the outpatient team

Knowledge, Skills, and Ability:

  • Ideal candidates will have a history of experience, education, and/or training in substance use, mental health, or the desire and commitment to gain the skills to provide behavioral health treatment services
  • Ability to engage individuals when they are asking for help and decrease barriers to accessing treatment
  • Ability to provide 8 meaningful client contacts within the guidelines of CCBHC per day. This may include client contacts including check ins when needed. One example of this could include when a therapist is out sick and the client asks for a check in for that day.
  • Ability to learn, practice, and support the learning of others in evidence-based treatment services
  • Organized and able to develop a program plan with goals, objectives, and timeframes that are consistently met
  • Energetic and internally driven to assist individuals in gaining linkage to necessary services
  • Knowledge of community resources and/or willingness to research what is needed for the client
  • Effective oral and written communication skills
  • Ability to work as a team player
  • Strong customer service skills
  • Ability to navigate electronic health record
  • Proficient in Microsoft Word and Excel

Education and Experience:

  • The successful candidate will minimally possess a Bachelors' degree in Human Services, Psychology, Social Work or related field, or have 5 years' working experience in Human Services.
  • Master's degree in counseling and guidance, rehabilitation counseling, social work, vocational counseling, psychology, pastoral counseling, or family therapy, as well as education/experience to be a QMHP (Qualified Mental Health Professional) is highly recommended.
  • Licensure as a CADC, LCSW, LCPC, LPC, or LSW is required within 2 years of employment.
  • Valid driver's license and reliable means of transportation are required for all Heritage positions.

Salary Range: $47,840-$56,160 per year (dependent on education and experience)
