Position Summary
As a key member of the care team, you will provide direct psychological, social, and practical support to a defined caseload of patients and families. Responsibilities include conducting social work assessments, identifying needs, setting goals, and implementing care plans. You will coordinate referrals to external agencies and ensure continuity of care, working in close collaboration with a multidisciplinary team to support optimal patient outcomes. This role also contributes to the achievement of broader organizational goals and requires flexibility to provide coverage during staff absences, vacancies, and on-call needs.

Education Requirements:




  • Must hold an LMSW (Licensed Master Social Worker) credential from the Maryland Board of Social Work Examiners.



  • Exceptions may apply for candidates licensed in another state or with comparable qualifications, pending approval from the Board and successful completion of the required examination(s).




Preferred Experience:




  • At least one year of experience providing clinical social work services, preferably in a mental health setting (internship experience may be considered).



  • Strong oral and written communication skills to effectively engage with hospital staff, external healthcare providers, patients, and families.



  • Demonstrated ability to collaborate with interdisciplinary teams.



  • Excellent organizational and case management skills, particularly suited to a fast-paced, dynamic environment.
