PRIMARY FUNCTION: The medical case manager (MCM) addresses the needs of medically-complex patients (i.e., patients who are consistently unable to come into the office for medical visits and routine lab work, have continuous, poor medication adherence, and/or identified to be high-need) in the ambulatory medical setting by: 1) coordinating all medical-related care and services, 2) identifying and resolving care barriers, and 3) facilitating receipt of social and supportive services to maintain optimal health. The aim is to allow patients to efficiently access all services with in-house resources.

Job Description

ESSENTIAL FUNCTIONS:
* Communicates with physicians and other professionals to develop a care plan that facilitates the efficient use of health care resources, and aids patient in executing plan.
* Develops outreach plans with co-workers (HIV social care team members and clinicians) in order to establish continuous follow-up with patients.
* Reviews pertinent medical history, current diagnosis, and pharmaceutical data via EMR including tracking medical data (e.g., checking whether patients have up-to-date labs).
* Assists patients in forming solutions for presenting problem, co-defines and determines realistic goals.
* Promotes wellness and provides education regarding preventive care measures, including facilitating health promotion group sessions.
* Documents client activities and coaching/counseling sessions in established clinical format.
* Interacts with co-workers, visitors, and other staff consistent with the iSCORE values of Jefferson.

EDUCATIONAL/TRAINING REQUIREMENTS:
Master's degree in social work, counseling, or a related field.

5 years of experience in a physical or behavioral health position
