The Medical College of Wisconsin's Department of Medicine - Division of Cardiology is seeking a Nurse Practitioner or Physician Assistant to provide primary or specialty medical care. The APP will collaborate with interdisciplinary healthcare teams in diagnosis & treatment of conditions requiring intervention in an outpatient environment as a member of the Cardiology Clinic located in Oak Creek, WI at Froedtert Drexel Town Square Health Center. The Advanced Practice Provider (APP) works as a member of a multidisciplinary health care team who will contribute to excellence in program administration, patient care, patient safety, research, quality improvement, & teaching. The Ideal Candidate will be an APP with strong communication skills, strong foundation in evidence-based practice, and ensure an environment of safe quality care, wanting to be part of a thriving and very collaborative academic practice at the Medical College of Wisconsin (MCW). Areas of responsibility will include, but not be limited to providing direct patient care in an outpatient setting. In this role, you will:
- Collaborate with interdisciplinary health care teams in diagnosis & treatment of conditions requiring Cardiovascular intervention in an outpatient environment
- Write accurate and timely progress notes, procedure notes, and/or admission & transfer orders
- Order, perform, & interpret screening and diagnostic procedures and tests, and appropriately communicate and document information
- Facilitate & communicate with patients, families, and staff to promote continuity of care across the continuum of care
- Attend required training, and meet key regulatory requirements, including those of the privileging institution, CMS, and Joint Commission
- Evaluate quality outcomes for the patients receiving care of this team through participation in the development of quality indicators & outcomes dashboard
- Acquire information on the most current development & trends in the Physician Assistant field and/or Nurse Practitioner field, through educational programs, in-services, professional associates, and current literature
- Collaborate with cross-department healthcare teams for management of cardiovascular disease. Serve as liaison with RNs to ensure continuity of core rounds,
- Provide cross-coverage on consult service
- Coordinate care coordination rounds
- Participate in ongoing clinical research projects related to cardiology
Core Advanced Practice Responsibilities:
- Perform comprehensive histories and physical exams, identify normal and abnormal findings, including physical, cognitive, and psychosocial status
- Order, perform, and interpret screening and diagnostic procedures and tests, and appropriately communicates and documents information
- Write accurate and timely progress notes, procedure notes, and/or admission and transfer orders
- Develop pharmaceutical and non-pharmaceutical treatment plans
- Monitor effectiveness of plan of care
- Educate patients on disease and treatment plans
- Perform procedures as trained and appropriately credentialed
- Document within the Electronic Medical Record (EMR) and complete other necessary documents
- Collaborate and/or consult with physicians and other health care providers as appropriate
- Educate, counsel, and assist patients and families regarding current health problems, health maintenance activities and preventive behaviors
- Attend required training and meet key regulatory requirements, including those of the privileging institution, CMS, and the Joint Commission
- Acquire information on the most current development and trends in the Physician Assistant field and/or Nurse Practitioner field, through educational programs, in-services, professional associates, and current literature
