The Community Health Worker assists in providing outreach and engagement with eligible Managed Care Plan members and supporting care management through transportation assistance to assigned appointments and social services. Shifts Available: Full Time; 8:30 am - 5:00 pm Monday - Friday :: SIGN ON BONUS $2000 Expected starting wage range is $21.00 - $23.24. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
