ABOUT THE JOB

MU Health Care is looking for an enthusiastic and knowledgeable Ambulatory Clinical Pharmacist to fill a PRN position and be a key player in our healthcare team. Working under collaborative practice, the ambulatory care pharmacist will be responsible for providing centralized anticoagulation (warfarin) management, disease state management within a family medicine clinic, and supporting population health quality initiatives including completing medication reviews and collaborating with providers to optimize medication therapy, improve medication adherence, promote wellness, prevent disease, and improve outcomes. You will play a crucial role in educating patients and serving as a valuable resource for prescribers. We value your pharmacist license in Missouri, and previous experience or ASHP accredited ambulatory care/community residency would be a great asset. DETAILED JOB DESCRIPTION

Demonstrate ability to review and verify medication orders to assure appropriateness utilizing clinical information in the electronic medical record. Prioritizes medication orders based on acuity and order status (i.e. STAT, NOW, routine).

Provide appropriate patient specific medication recommendations that ensure safety, efficacy, and cost effectiveness.

Assist in education and training of pharmacy students and pharmacy technicians, and residents.

Serve as a resource for the pharmacy team, nursing, and prescribers for questions/issues related to pharmacotherapy; analyze literature to support usage and contact prescribers as needed.

Compound and dispense pharmaceutical preparations which includes, but are not limited to compounded sterile products (IV solutions, injectable medications, and irrigation solutions), oral products (solutions, suspensions), and topical preparations (ointments, creams).

REQUIRED QUALIFICATIONS

Possession of a license to practice as a Pharmacist in the state of Missouri is required.

Previous experience in hospital pharmacy and/or the completion of an ASHP Accredited Residency is preferred.
