Job Overview: As part of RWJ Barnabas Health Medical Group, you will play an instrumental role in providing support to practices in achieving a model for improving and managing performance in various Value-Based Programs. The Patient Navigator s main objective is to support the Medical Group to efficiently deliver value to patients and physicians while operating as a high-performing academic medical group within the RWJBH One System-One Family Model. The Patient Navigators will work directly with the Primary Care Service Line to align transformation activities and allow for quick implementation and dissemination of information to practices.
