TheManager, Practice Engagement, will be a part of the Practice Engagement lane on Clinical Transformation within DaVita Integrated Kidney Care (IKC). The Clinical Transformation team leads DaVita in the transformation of kidney care by enabling care teams to engage patients in reaching their health goals and reducing overall cost to the healthcare system. Being the Partner of Choice to nephrologist practices allows our care teams to improve the clinical outcomes for our patients and grow this line of business. The Practice Engagement lane seeks to strengthen these practice partnerships through practice collaboration, reporting, technology, innovative care models, etc. This Manager will drive the development, implementation, and continuous improvement of such initiatives. The Practice Engagement lane is highly visible and works cross-functionally with stakeholders across the organization, including field operators, clinical, financial, IT, and compliance. The team is made up of highly motivated individuals with consulting, process engineering, change management, and clinical experience. Essential Duties and Responsibilities:
