The associate is responsible for providing individual substance abuse counseling, group counseling, psychosocial evaluations, and case management to patients enrolled in the Chemical Dependency Program. Responsibilities: Screen patients to ensure appropriateness for care Complete biopsychosocial evaluation utilizing all available data, including interviews with patients and families and conferring with interdisciplinary associates. Develop, implement, and validate treatment plans/discharges in a timely manner in collaboration with patient, family and interdisciplinary team. Provide chemical dependency counseling to individuals, groups, couples and families. Document client progress as delineated in program policies and procedures and regulatory bodies. Participate in staff conferences and other meetings. Participate in relevant performance improvement activities, including trainings and conferences. Provide community outreach and education. Qualifications: Bachelor's degree required CASAC or license in social work, mental health counseling or a related field is also required Minimum 1 year work experience as an addictions counselor (preferably with methadone clients) required Knowledge and ability to integrate historical and current patient issues Knowledge of substance abuse counseling and treatment Effectively work with adults and older adults with addiction The hours for this position are Monday 11:00 am - 7:00 pm and Tuesday - Friday: 8:30 am - 4:30 pm This is a 100% grant funded position Montefiore Medical Center is an equal employment opportunity employer.
