Job Overview:

The HIM Technician II is a cross-trained professional responsible for a wide range of functions within the Health Information Management (HIM) Department. This role ensures the integrity, accuracy, and quality of both paper and electronic medical records while supporting compliance with organizational standards and regulatory requirements. Key responsibilities include performing chart quality control, transcription, queries, analysis of chart deficiencies, prepping, scanning, boxing, and chart correction, as well as managing audits, suspension processes, and processing commitment papers.

The HIM Technician II also handles birth certificate preparation, death certificate validation, and Epic queue management. In addition, this role follows up with physicians on query completion, reconciles deficiency reports, and maintains aging and suspension lists to ensure compliance. The position ensures operative reports and histories & physicals are dictated and available within 24 hours of surgical procedures. The HIM Technician II assists and educates new physicians on medical record completion requirements, supports the HIM Data Analyst with various job functions, and provides clerical support as needed, including chart assembly, scanning, and storage.

Qualifications:

Required:

  • Minimum education of a High School diploma or equivalent
  • Minimum of three years clerical experience, preferably in a Medical Records department
  • Ability to identify and address quality concerns that jeopardize the integrity of the medical record and work to resolve them
  • Exceptional communication skills
  • Great attention to detail
  • Collaborative attitude

Scheduling Requirements:

  • Full-Time, Day, Hourly

Essential Functions:

  • Ensures receipt of all inpatient/outpatient records with follow up for all missing records
  • Assembles/preps and analyzes medical records per hospital guidelines for chart order
  • Cover batch sheet to be generated and printed and placed on charts, indicating discharge date and date scanned and by whom
  • Works with providers to resolve questions or concerns surrounding chart completion/documentation
  • Verifies and corrects date of death
  • Audits/release to Inspector and review and approve
  • Ensures admission assessments are completed within 24 hours and notifies provider
  • Daily creation and distribution of suspension lists/notices including daily OR Booking hold
  • Accurately prepares records for pick-up
  • Monitors and ensures MMF, On-Base and Epic Work Queues are up to date
  • Ensures time corrections are done timely upon receipt
  • Process commitment papers upon receipt
  • Ensures all documents are scanned timely upon receipt
  • Meets and sustains productivity standards
  • Refer all unresolved issues to Operations Manager
  • Verifies patient name, medical record number and patient account numbers on all sheets to ensure correct reports are in correct patient chart and on correct admission
  • Provides a compassionate and supportive environment with prompt and consistent service
  • Adheres to patient rights: provides for the confidential treatment of all communications and records
  • Demonstrates professionalism with families, visitors, physicians, co-workers and supervisors
