Job Overview:

The Community Outreach Coordinator plays a vital role in advancing the organization's mission to build healthier communities by improving health equity and access to care. This position is both a front-line and administrative position. The position performs a wide variety of duties including screenings, engaging, supporting, advocating, and educating community members on JCMC services while also tracking and reporting out our community outreach efforts.

The role requires a passionate, community-focused professional with excellent interpersonal skills and the ability to navigate diverse populations.Working under the direction of the AVP, Hospital Outpatient Services and Social Impact, the Community Outreach Coordinator ensures that all outreach and engagement activities align with RWJBarnabas Health's Community Health Needs Assessment (CHNA), Community Health Improvement Plan (CHIP), and strategic priorities.

Qualifications:

Required:

  • High School Diploma Required
  • Previous experience in community engagement, outreach coordination, or community-based health program delivery
  • Experience working with diverse populations and addressing social determinants of health.
  • Must have a valid driver's license and access to reliable transportation.
  • Willingness to work flexible hours, including evenings and weekends, as needed for community events and engagement.

Preferred:

  • Associate or bachelor's degree in public health, Social Work, Community Development, or a related field preferred.
  • Medical Assistant Certification Preferred

Essential Functions:

  • Responsible for the coordination of JCMC outreach events and requests. (Coordination may include attendance of events, securing volunteers for events, securing educational materials, giveaways etc.)
  • Participation and attendance in community outreach events is required. (Weekends required)
  • Build and maintain collaborative relationships with community-based organizations, local leaders, faith-based institutions, schools, and government agencies.
  • Coordinate and implement health outreach initiatives such as screenings, educational workshops, health fairs, and vaccination drives.
  • Coordinate and promote attendance at outreach events that enhance community visibility and participation.
  • Represent RWJBarnabas Health at community meetings, events, and coalitions to promote services and resources.
  • Maintain an up-to-date inventory of RWJBarnabas Health services, local health and social services resources to provide referrals and support navigation efforts.
  • Assist individuals in accessing RWJBarnabas Health services and other community resources such as primary care, behavioral health, and social services.
  • Assist in the dissemination of culturally and linguistically appropriate health education materials.
  • Collect and maintain accurate data on outreach activities, participant feedback, and community needs. (Data to include attendance, health outcomes)
  • Document outreach activities, community interactions, and program outcomes using standardized reporting tools.
  • Support reporting for grants, funding agencies, and internal evaluations.
  • Contribute to Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP) activities.
  • Responsible for tracking and presenting outreach data at monthly JCMC Community Outreach/ Health Equity meetings.
  • Responsible for purchasing and tracking inventory of departmental supplies.
  • Collaborate with departments including Population Health, Marketing, Behavioral Health, and Clinical Services to coordinate outreach messaging and service promotion.
  • Attend regular staff meetings, trainings and other meetings, as requested.
  • Supports and promotes effective patient engagement by demonstrating courteous and cooperative behavior when interacting with visitors; maintains confidentiality of work-related issues and performs other duties as required or assigned.
  • Promotes a positive and healthy working environment.
  • Performs other duties as assigned.
