DESCRIPTION OF WORK

In this role, you will assess clients' needs, develop tailored service plans, and deliver direct interventions to improve their overall well-being. You will act as an advocate for those you serve, working collaboratively with various agencies to connect clients with necessary resources and programs.

Serving as a member of the interdisciplinary care plan team, where you will engage in meetings and ensure that all documentation is thorough and accurate. Delivering direct social work services to residents facing behavioral and psychosocial challenges. This involves arranging or providing behavior management services tailored to meet the emotional and behavioral needs of residents, conducting behavioral assessments, and implementing person-centered interventions for those requiring specialized support.

You will also play a key role in educating residents about their rights, assisting with advanced directives for healthcare, investigating grievances, and discussing voting status with residents. Furthermore, you will provide counseling to residents and their families, either individually or in groups, utilizing various therapeutic approaches such as supportive counseling, grief counseling, family therapy, crisis intervention, and behavior modification to help them identify challenges and explore strategies for enhancing their personal and social functioning.

Work Schedule and Additional Information:

  • Full-time employment, 37.5-hour work week
  • Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
  • Telework: You will not have the option to telework in this position.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Two years of experience as a Social Worker 1 or Forensic Social Worker 1 (commonwealth titles) (must have been employed in a commonwealth social worker job title on September 8, 2008, with continuous commonwealth employment); or
  • Two years of experience as a Social Worker 1 or a Forensic Social Worker 1 (Commonwealth job title or equivalent Federal Government job title, as determined by the Office of Administration) and a bachelor's degree in social work or social welfare; or
  • Two years of professional experience providing direct public or private social work services, and a bachelor's degree in social work or social welfare; or
  • A master's degree in social work or social welfare.
  • Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree.

Special Requirement:

  • This position requires an active license as a Social Worker or Clinical Social Worker issued by the Pennsylvania State Board of Social Workers, Marriage and Family Therapists and Professional Counselors.

  • A conditional offer of employment may require satisfactory completion of a medical examination and drug screening.
  • This position falls under the provisions of the Older Adult Protective Services Act.
    • Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
