The Physical Therapist is a qualified professional who directs, supervises, evaluates, and provides direct care to patients and provides physical therapy services to patients in the home according to State and Federal regulations under the direction of the attending physician.

Responsibilities

  • 1. Comprehensive evaluation of the patient's level of function and development of the therapy plan of care.
  • 2. Participation in case management conferences and documentation thereof.
  • 3. Instruction of patient, caregivers, Physical Therapy Assistants (PTAs), and other agency clinical staff in the patient's treatment plan, including follow-up supervision.
  • 4. Selects and administers diagnostic and therapeutic techniques and materials.
  • 5. Supervision of PTAs. Confers with PTAs at least weekly and when there is a change in condition.
  • 6. Supervision of Home Health Aides when they are participating in the patient's therapy care plan.
  • 7. Documentation is completed in the EMR within 24 hours after visits are made.
  • 8. Assists patients and caregivers in obtaining necessary equipment and provides instruction on how to use equipment.
  • 9. Responsible for maintaining productivity and appropriately communicates needs for additional visits when necessary.
  • 10. Ensures that there is insurance authorization prior to making visits when appropriate.
  • 11. Reports incidents that occur to the RN case manager and the Manager, Therapy Services.
  • 12. Participates in staff meetings, in-service programs, and other meetings as requested.
  • 13. May perform other duties as assigned.

Qualifications

EDUCATION

Bachelor of Science in Physical Therapy required, masters or doctorate desirable.

EXPERIENCE

Minimum of two (2) years of clinical experience in Physical Therapy in an acute or sub-acute care setting. Home care experience is preferred.

LICENSURE

Physical Therapist with valid (or eligible) Connecticut license. A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day. CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within 3 months (90 days) of hire.

SPECIAL SKILLS

Excellent written and verbal communication. Ability to work collaboratively with multidisciplines. Must be adaptable, flexible, able to learn quickly, use critical thinking skills, and oversee PTAs. Strong computer skills.

PHYSICAL DEMAND

Ability to perform tasks involving physical activity which may include heavy lifting and extensive bending and standing. Requires exposure to blood and body fluids, hazardous materials and communicable diseases.
